Individual application

To apply for individual membership, click on the link below to download the Individual Membership application form. Once you’ve downloaded the form – complete it and return it to NHP with the following documentation:

  • ID/Passport of principal member;
  • Copy of birth certificate of children (if applicable);
  • Marriage certificate when registering a spouse;
  • Proof of previous medical aid fund (if applicable); 
  • ID/Passport of spouse (if applicable);
  • Dr’s certificate.

 

When completing the form kindly remember to indicate the required date of membership. This date can only be from the 1st day of the present calendar month, or future date, but not more than 3 months in advance. You can use the Benefit Calculator to determine your pro-rated benefits, according to your desired date of membership.

 

The application form will also require that you select an NHP option, if you have not decided on an option yet, make use of our online suggestion tool to assist you in the decision making process. It is also important to remember that the benefit option selected includes the following inclusive benefits (of which the risk is fully underwritten by a registered insurer, as required by the Medical Aid and Insurance Acts):

 

Once the application form has been completed you can submit it to any of NHPs branches throughout Namibia, go to the Contact Us section for branch information.
